DESCRIPTION
Salbu (Salbutamol) is a directly acting sympathomimetic drug, that restricts the action to Beta2-adrenergic receptors. Its broncho-dilating action is more prominent than its effect on the heart, therefore salbutamol is used for the management of asthma.
MODE OF ACTION
Salbutamol stimulates beta2 receptors in airways smooth muscle, which causes bronchodilation.
PHARMACOKINETICS
Salbutamol is readily absorbed from the gastrointestinal tract, undergoes first-pass metabolism in the liver, excreted in the urine half as an inactive conjugate and the rest as unchanged salbutamol. The plasma half-life of salbutamol ranges from 2-7 hours. After administration, action started within 15 minutes and sustained for 4 hours.
INDICATION
1. Bronchial asthma, status asthmaticus and allied conditions concerned with reversible bronchospasm.
2. Prevention of premature labour
DOSAGE AND ADMINISTRATION
Adult: 4 mg 3-4 times daily. Elderly and sensitive patients should be given 2 mg initially. Maximum single dose 8 mg (but unlikely to provide much extra benefit or to be tolerated). Children: 2-6 years: 1-2 mg 3-4 times daily. 6-12 years: 2 mg 3-4 times daily.
CONTRAINDICATION
Hypersensitivity to salbutamol contraindicates the use of Salbu.
USE IN PREGNANCY AND LACTATION
It should used in the first trimester only if absolutely essential. In lactation, can be used.
SIDE-EFFECTS
These are usually trivial, consisting mainly of tachycardia and tremor. Headache and peripheral vasodilation may occur in large doses.
WARNINGS AND PRECAUTION
Salbutamol should be used with caution in patients with hyperthyroidism, diabetes mellitus, and severe cardiovascular disorders of hypertension. In asthmatic patients whose condition deteriorates despite salbutamol therapy, the dosage should not merely be increased, but alternative or additional therapy including corticosteroids should be instituted promptly.
DRUG INTERACTION
Beta-antagonists inhibit the action of salbutamol.
