Presentation
- Perilac 10 mg tablet: Each film coated tablet contains Ketorolac Tromethamine USP 10 mg.
- Perilac 30 mg IM/IV Injection: Each ampoule contains Ketorolac Tromethamine USP 30 mg.
- Perilac 60 mg IM/IV Injection: Each ampoule contains Ketorolac Tromethamine USP 60 mg.
Description
- Perilac is a potent analgesic agent of the non-steroidal, anti-inflammatory class (NSAID). It inhibits the cyclo-oxygenase enzyme system and hence prostaglandin synthesis. Perilac is not an anesthetic agent and possesses no sedative or anxiolytic properties.
Indications: Perilac IM/IV and oral are indicated for
- Post Operative Pain
- Post Partum Pain
- Acute Dental Pain
- Traumatic Pain
- Musculoskeletal Pain
- Acute Renal Colic
Dosage and administration
Perilac IM/IV
- Adult patients: Perilac IM/IV may be used as a single or multiple dose on a regular or prn (when necessary) schedule. The IV bolus must be given over not less than 15 seconds. The IM administration should be given slowly and deeply into the muscle.
Single-dose treatment:
Adult patients: IM dosing:
- Patients <65 years: One dose of 60 mg
- Patients >65 years with renal impairment: One dose of 30 mg.
Adult patients IV dosing:
- Patients <65 years: One dose of 30 mg.
- Patients >65 years with renal impairment: One dose of 15 mg.
Pediatric patients (2 to 16 years):
- IM dosing: One dose of 1 mg/kg up to a maximum of 30 mg and IV.
- dosing: One dose of 0.5 mg/kg up to a maximum of 15 mg.
Multiple dose treatment:
- Patients <65 years: 30 mg every 6 hours, up to maximum 120 mg daily.
- Patients >65 years renal impairment: 15 mg every 6 hours, up to maximum 60 mg daily.
Transition from Perilac IM/IV to Perilac tablet in adults:
- Patients <65 years: 2 tablets as a first oral dose for patients who received 60 mg IM single dose, 30 mg IV single dose or 30 mg multiple dose, Perilac IM/IV followed by 1 tablet Perilac oral every 4 to 6 hours, not to exceed 40 mg/24 hours of Perilac oral.
- Patients >65 years renal impairment: 1 tablet as a first oral dose for patients who received 30 mg IM single dose, 15 mg IV single dose or 15 mg multiple dose. Perilac IM/IV followed by 1 tablet Perilac oral every 4 to 6 hours, not to exceed 40 mg/24 hours of Perilac oral.
- Duration of treatment: In adults, the maximum combined duration of use (parenteral and oral Perilac) is limited to 5 days.
Contra-indications
- Perilac is contra-indicated in patients with a history of peptic ulcer or gastrointestinal bleeding,
- cardiovascular bleeding, coagulation disorders, patients with hypersensitivity to ketorolac or other NSAIDs, angio-edema or bronchospasm, a history of asthma, during pregnancy, labor, delivery or lactation, children under 16 years of age.
Precautions
- Cautions should be exercised with Perilac treatment in patients with kidney or liver disease, patients with a history of peptic ulcers and bleeding, It may cause liver inflammation or kidney problems in some people. Perilac can increase water retention. If you have heart disease or high blood pressure, use this drug with care. This medication can prolong bleeding time. If you are taking blood-thinning medication, take Perilac with caution.
Side-effects
- More common side effects may include: Diarrhoea, dizziness, drowsiness, headache, indigestion, nausea, stomach and intestinal pain, swelling due to fluid retention
Pregnancy, nursing mothers:
- The safety of Perilac in human pregnancy has not been established. Perilac is therefore contraindicated during pregnancy, labor or delivery, in mothers who are breast-feeding.
Drug Interactions:
Concurrent treatment with probenecid, oxpentufylline, lithium is contraindicated. Perilac should not be used with other NSAIDs. Therapeutic concentrations of digoxin, warfarin, paracetamol, phenytoin and tolbutamide do not alter ketorolac tromethamine protein binding. There is an increased risk of renal impairment when ketorolac tromethamine is administered concurrently with ACE inhibitors, particularly in volume-depleted patients. Caution is advised when methotrexate is administered concurrently.
